Selection Day Summer Programs

Summer Programs Selection Day
Please complete the for before the Selection Day.
Please print the first 2 pages and Select the order of preference for the programs taking in consideration the YEAR your child was born (not the age they will be during the camp), select the option of payment and finally, pick 5 dates that you can chaperone the dances (on the back of the page).
Once this is done make sure you sign the document and send a copy back to the chapter email:
